Paperwork for new patients
Make sure that you complete your packet of paperwork and send it to us via fax or mail before your first visit.
- If you are scheduling your first appointment for headaches or migraines with Dr. Factor, Dr. Klein, or Dr. Li, fill out the Headache Packet. Otherwise, download the standard New Patient Packet.
- If you are a caregiver for an Alzheimer’s/dementia patient, fill out the Caregiver Info Form as well.
